OSFC SFA Forms for Electronic Payments
Requirements for Credit Card Payments.:
- Certification Forms:Please have a copy of your certificate(s) and/or invoice ready to upload which is required for some of the payment request processes.
- Resident Forms:Please have a copy of your invoice ready to upload when making payments.
The Forms can be used for Mail-In Applications as well. See the details below:
- Please complete the form and Select Payment Type as "Mail-In Application"and then click "Submit" button.
- Enclose a check or money order with the printed application (Payable to Commonwealth of PA).
- Do Not add documents, Please mail with your completed form and fee.
